Residential Homes (Rule 24D)

PIR II operates 2 Residential Homes. We serve adults diagnosed with developmental disabilities and/or serious and persistent mental illness. Most funding is provided through the DD and CADI waiver.

Our homes are staffed by compassionate professionals trained in the areas person person-centered planning provision of service as well as understanding and providing skills and support in coping with mental illness, developmental disabilities, and the rules and regulations set forth by DHS Rule 245D. Support staff is also trained in CPR, First Aid, Medication Administration, Vulnerable Adults Reporting, Maltreatment of Minors, HIPPA, and Person-Centered Principles. PIR II uses the person-centered approach to assist individuals in meeting their life vision. Program plans are developed by the individual, in conjunction with their team members focused on desired outcomes. Increased independence, making responsible choices, skill development, coping strategies, housing support, employment searching, and support, family and community involvement are most often the main focus.

Our core support staff have provided services together for 10 years, which has enhanced the provision of consistent support and continuity of care.

Support staff communicate with social workers, case managers, legal guardians, medical providers, community members, and other programs to facilitate the continuation of care.
